Pathology billing can often be burdened with complications when confronting various encounter types.

There are numerous CPT codes and conditions to deal with in order to correctly submit your claims.  Depending on your practice, you may need to bill for hospital based services as well referred specimens from other hospital or private practices.
Practicing pathology providers perform multiple specimen testing’s on a given patient which will generate an abundance of claims, involve multiple payors and can result in a myriad of claims issues.  Having an experienced medical billing team, with visibility into performance, is vital to your practice’s financial health.

  • Pathology billing breaks down into several types of work being performed. Some examples are:

    • Clinical Pathology

    • Anatomical Pathology

    • Drug Testing

    • Hematology

  • For Surgical pathology, different specimens submitted for separate and individual evaluation are often billed using CPT codes for each specimen and on the same collection date. Due to this, modifiers are used to identify this. Proper use of the modifiers will help to minimize denials.

  • Coders need to monitor the correct use of “add on“ codes as there are a number of these in the “Pathology’ section of the CPT 4 Manuel.

 

Here are some of the complications that are affecting Pathology revenue collection procedure:

  • Omitting modifiers causes denials for “duplicate” tests.

  • Billing more than the maximum units per date of service. Medicaid fee schedules will only reimbursed up to a certain number of units

  • Billing “add on” codes alone

  • Annual review and update of your fee schedule so as to add new CPT codes deactivate deleted codes, checking to be sure that your fees are priced correctly so as to maximize your payments.

  • Keeping track of the correct CLIA numbers that must be submitted on the CMS 1500 forms as well as recognizing which procedure codes are considered “CLIA Waived”.

On October 1, 2016 the Centers for Medicare and Medicaid Services (CMS) will no longer accept unspecified codes in most cases. 

Starting in October, CMS requires greater specificity for claims filed in ICD-10-CM/PCS. The 12-month grace period originally introduced in 2015 said they would not deny claims for lack of ICD-10 specificity. That all changes on October 1, 2016. Payors will most likely follow suit if they haven't already.
